Cancellation Policy

### Rooted & Restored Wellness **Booking & Cancellation Policy — Initial Consultations & Memberships** **Appointment Reservations** All appointments are available by advance reservation and subject to availability. Full payment for standalone initial consultations or the first monthly membership payment is required to secure your appointment. **Membership Appointments** Active members may schedule appointments included in their membership plan at no additional appointment charge. Membership benefits are available while the membership is active and payments are current. **Cancellations & Rescheduling** We kindly request at least 24 hours' notice for all appointment cancellations or rescheduling. - **Standalone Appointments:** Cancellations made with less than 24 hours' notice or missed appointments are subject to a $50 cancellation fee. - **Membership Appointments:** No cancellation fee applies to appointments included in an active membership. We still appreciate advance notice so the appointment time can be offered to another client. **Late Arrivals** Please arrive on time. Late arrivals may result in a shortened appointment or require rescheduling, depending on availability. **Initial Wellness Consultations** Initial consultations include a review of health history, current concerns, wellness goals, and individualized recommendations. Laboratory testing, supplements, and additional services are not included unless specifically stated in the selected membership plan. **Clinical Services** All clinical recommendations and treatments are determined by the licensed medical provider following an individualized assessment. Booking an appointment does not guarantee any particular treatment or prescription.
